Nurse Coordinator Overview:
Edison Home Health Care is seeking a Nurse Coordinator to join our home care agency's Nursing team. The Nurse Coordinator will primarily work from our Brooklyn office and play an integral role in the department. Pulling up Initial Report/Start of Care daily Running Discharge Report daily Running Reassessment Report weekly Assigning RNs to complete visits Following up with RNs on the first day of service to remind them of upcoming visits and confirm completion Following up on RN paperwork Saving all documents in the patient folder Entering POC in exchange Sending out MD Orders Sending HHA supervision to HR Completing the Quality Survey JOT form Mailing POC to patients if needed Assisting with authorization for visits as necessary Using and understanding the current software systems Assigning incident visits to RNs received via smart sheets Requirements:
High school diploma or equivalent Experience in healthcare/home care (preferred) Knowledge of HHA Exchange (preferred) Proficiency with Microsoft Office, especially Excel Effective communication skills with employees, patients, and families Problem-solving and data collection abilities Benefits include: Direct Deposit Health Insurance 401K PTO and Sick Days
