Job Description Summary: Job Description: As an Eastern Region Float Provider for Community Health Center, you will: Provide a full scope of medical services largely geared to underserved populations Receive strong clinical support services and work in an innovative practice environment utilizing Advanced Access scheduling Be part of a multi-disciplinary practice that offers a blend of individual and population based care What would the schedule look like? Based on-site at a CHC site that need staffing support to help cover PTO, various leaves, etc. for a minimum of 3 days a week. There is also opportunity for
telehealth/telework
for the remainder of the week (1-2 days depending upon on-site need). Our Eastern Region consists of: Enfield Hartford Middletown New Britain Clinton New London Groton APRN Qualifications: Graduate of an accredited Master of Science in Nursing program with a specialty as a Family Nurse Practitioner ANA certification as a Family Nurse Practitioner State of Connecticut APRN and RN licensure (within 12 months) required. Bi-Lingual Skills English/Spanish preferred Certifications Required: Medical - State of Connecticut Licensure Physician Assistant Qualifications: Graduate of an accredited Master program in the study of physician assistant. Maintains appropriate board certification, licensure, and professional organizations. Bi-Lingual Skills English/Spanish preferred Organization Information: The Moses/Weitzman Health System is a global leader addressing challenges faced by organizations caring for the poor and diverse populations, and is home to programs focusing on education, research, and process improvement support for safety net providers.
