Graduate of state approved School of Nursing. Licensure/Certifications
Current RN license in the state of Louisiana. Training/Experience
Previous experience preferred, but not required. Responsibilities
Be available by telephone during working hours. Act as liaison between patients, physicians, and referral sources. Be knowledgeable of all assigned patients. Conduct ongoing clinical reviews to ensure appropriate level of care and necessity of continued stay. Report clinical reviews to third-party payer sources as required/requested. Post authorizations into computer as needed. Make appropriate referrals as ordered per MD. Ensure the most effective and appropriately leveled discharge plan for all assigned patients to promote safety in the post-acute setting. Actively participate in interdisciplinary rounding meetings. Comply with all compliance expectations. Be knowledgeable of CMS mandated forms. Benefits
