Pay Range: $125,000 - $130,000 Per Year Description
Supervise daily operations of interdisciplinary teams and participant outcomes. Coordinate multidisciplinary integrated care across various centers and care continuum. Ensure completion of metrics logs and provide necessary support. Facilitate care planning meetings and mediate disagreements on patient care treatments. Oversee development and execution of personalized care plans for improved outcomes. Participate in quality assurance and regulatory compliance activities. Investigate and resolve customer issues and grievances promptly. Key Responsibilities
Oversee and support site-based managers and clinical staff. Coordinate care planning meetings and follow-ups. Allocate resources and ensure adequate team coverage. Implement in-service training and development directives. Collect and review operational metrics data for quality assurance. Monitor and minimize voluntary disenrollments. Ensure documentation is complete, accurate, and timely. Qualifications
Baccalaureate degree in Business Administration, Healthcare Administration, or related fields. Degree from an NLN accredited school of nursing, BSN preferred. Minimum of three years of administrative experience in a management capacity in healthcare settings. Customer Service Experience Required. Current active and unrestricted license and registration in New York State as a Registered Nurse. Proficient in computer use for email, word processing, and maintaining participant records.
