Urgently Hiring Home Health and Hospice RNS - Apply Now! Join Amergis Healthcare Staffing for a role delivering coordinated nursing care in patients' place of residence, following established standards of care and the nursing process.

Responsibilities

  • Deliver coordinated nursing care in the patient’s home according to established standards and the nursing process.
  • Supervise and direct activities of various levels of assigned nursing staff; coordinate care with other disciplines.
  • Collaborate with patients, families, and healthcare providers to ensure a safe, healing, and compassionate environment.
  • Assess and document learning needs, provide education to patients and families, and complete all Oasis documentation accurately.
  • Delegate appropriately and coordinate duties of the healthcare team; perform other job-related duties as assigned.

Qualifications and Experience

  • Bachelor's Degree in Nursing (BSN): Preferred
  • One year of professional nursing experience: Required
  • Acute care facility or home health experience: Preferred

Licenses and Certifications

  • Registered Nurse (RN) licensure in the state of practice: Required
  • Basic Life Support (AHA) certification: Required
  • Valid Driver's License and must be at least 21 years of age: Required

Essential Functions

  • Collects relevant data, analyzes assessment information, develops a plan, implements care, coordinates delivery, and evaluates progress toward outcomes.
  • Collaborates with the patient, family, and healthcare team to provide safe, humane, and caring care; provides health information to patients and families.
  • Assesses patient/family learning needs and documents understanding; ensures Oasis documentation is accurate.
  • Delegates tasks appropriately and coordinates duties of team members.
  • Performs other job-related duties as assigned.
