The Senior Licensed Vocational Nurse (LVN) plays an essential role in supporting UCSF's primary care model by providing high-quality, patient-centered clinical care across multiple UCSF medical center sites. Under the supervision of the UCSF Primary Care Administrative Nurse Manager, this position uses the nursing process to assess and identify individual patient needs, collaborate with care teams, and deliver treatments and education in alignment with the Patient-Centered Medical Home Standards and Ten Building Blocks of Primary Care. The Senior LVN works closely with healthcare providers, nurses, and medical assistants to ensure smooth clinic operations and superior patient experiences. Responsibilities include medication administration, lab result monitoring, procedures, and patient education for both pediatric and adult populations. Responsibilities
Clinical Care & Assessment: Assess patient needs through observation of symptoms and emotional and social factors. Administer medications, vaccines, and treatments per provider orders. Perform procedures such as wound care, ear irrigations, catheterizations, and phlebotomy. Conduct Point of Care testing and ensure proper documentation in the electronic medical record (ApeX). Patient Education & Advocacy: Provide patients and families with clear information about treatments, procedures, and available resources. Use interpreters and teaching materials to ensure patient understanding. Participate in patient action planning and health coaching initiatives. Coordination & Communication: Support patient flow and continuity of care through effective coordination and communication with healthcare team members. Participate in daily team huddles to plan patient-centered care. Maintain thorough and accurate patient documentation. Quality & Safety: Comply with all UCSF policies and standards for infection control, safety, and confidentiality. Respond appropriately in medical emergencies and participate in safety drills. Participate in performance improvement initiatives and training. Professional Development: Maintain required licenses and certifications (LVN, CPR). Attend classes and meetings to support professional growth and clinical excellence. Demonstrate UCSF's PRIDE values and AIDET communication principles in all interactions. Required Qualifications
Active California Licensed Vocational Nurse (LVN) license AHA CPR certification Three (3) years of LVN experience Trained and licensed to perform phlebotomy Strong communication and problem-solving skills Proficiency with electronic medical records (Epic/ApeX preferred) Ability to work in a fast-paced clinical environment Flexibility to orient and work at all UCSF primary care clinic locations Preferred Qualifications
Multilingual skills in Spanish, Chinese, or Russian Experience applying Lean principles in healthcare settings License/Certification
