Overview Director of Case Management – Alan B. Miller Medical Center

This is a full-time position (40 hours per week) with full benefits.

Responsibilities

Assumes responsibility and accountability for the fiscal, operational and personnel management of the Case Management Department, including RN Case Managers and licensed social workers.

Ensures achievement of inpatient and observation patient length of stay goals and appropriate patient status determinations.

Collaborates with the Chief Medical Officer and Medical Staff on Case Management/Physician issues.

Coordinates safe discharge processes for all patients to the appropriate next step in the continuum of care.

Develops plans to effectively manage patients post-discharge to prevent readmissions.

Leads the Hospital's Utilization Management Committee with the Medical Staff.

Ensures appropriate patients are referred to EHR for second-level patient status reviews.

Co-chairs the Hospital's Denials Committee with the Patient Access Director to prevent patient denials and post-discharge governmental and insurer take-backs.

Job Requirements

Current RN license in the State of Florida.

Case Management certification.

Bachelor’s degree required; A.B.S. or M.S. strongly preferred.

Five (5) years of Case Management Director experience in an acute care setting.
