Job Summary Evaluates and treats patients to restore functions based on physical therapy principles and practices. Participates in program development, staff and community education. Demonstrates leadership and independence. Provides service to all age groups, from infancy to geriatrics. Performs home environmental assessments, rehab care plan development, serves as technical specialist on daily tasks and cases, models, mentors and trains staff on technical skills.

Lead and work independently in the environment.
Provide service to patients across all ages.
Conduct home environmental assessments, assist with rehab care plan development.
Serve as technical specialist on daily tasks and cases.
Model, mentor, and train staff on technical skills.
Job Requirements
Bachelor’s Degree in physical therapy or equivalent.
Master’s Degree (preferred).
Three years of technical experience (preferred).
Two years of experience in inpatient acute care, acute therapy or outpatient setting (preferred).
Licenses & Certifications
Physical Therapy licensure in state of practice.
Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification.
Physical Therapist (PT).
Essential Functions
Accurate and complete patient condition documentation and notify physicians when warranted.
Provide assessments, evaluations, plans of care and treatments aligned with patient needs.
Develop work assignments and supervise PT assistants, technicians and aides.
Maintain and care for equipment and supplies, document maintenance and repairs.
Provide patient instructions about therapeutic procedures to be continued.
Perform other job-related duties as assigned.
Organizational Requirements Require all associates receive required vaccinations as a condition of employment and annually thereafter. Medical and religious exemptions may apply.
