Interdisciplinary Team (IDT) Coordinator Under the leadership of the Director of Administration and Operations and under the direct management of the PACE Center Manager, the Interdisciplinary Team (IDT) Coordinator is a critical member of the Neighborhood PACE organization. The IDT Coordinator provides coordination and planning support to all 11 IDT members that represent the various PACE service areas and clinical disciplines. The IDT Coordinator coordinates team meetings to facilitate patient planning and Care Plan creation. The IDT Coordinator will assume responsibility for PACE facility management and oversight in the absence of the Center Manager.

Position Summary The IDT Coordinator should be well versed in regulatory requirements related to PACE, the IDT, assessment, and care planning to ensure the model of team-based care provided by the IDT is compliant with applicable policies and procedures. In partnership with the PACE Center Manager, the IDT Coordinator reviews and analyzes discipline-specific assessment information to ensure compliance with PACE regulations.

Responsibilities

Provide coordination and planning support to all 11 IDT members representing various PACE service areas and clinical disciplines.

Coordinate interdisciplinary team meetings to facilitate patient planning and Care Plan creation.

Review and analyze discipline-specific assessment information in partnership with the PACE Center Manager to ensure compliance with PACE regulations.

Assume responsibility for PACE facility management and oversight in the absence of the Center Manager.

Qualifications Education: College degree in a field such as Rehabilitative Therapy, Nursing, Social Work, Recreational Therapy, Health Care Administration, or related work experience is required.

Experience: Prior experience in interdisciplinary team setting preferred. Supervisory and administrative experience is preferred.

Skills/Abilities: Maximum flexibility and excellent organizational skills; excellent attention to detail; ability to successfully facilitate team processes and communications; ability to prioritize competing requirements.

Pay

$60,840 - $88,452/year based on experience.
