Social Services Director Position Summary The Social Service Director is responsible for planning, organizing, developing, and directing the overall operation of the Social Services Department. This must be done in accordance with current federal, state, and local standards, guidelines, and regulations, our established policies and procedures, and as directed by the Administrator. The Social Services Director interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process. This position is responsible for patient assessments, care planning, and supervising additional social services staff. Assists in the development of policies regarding participation in facility planning for health and welfare services. Provides input to strategic decisions as related to social services areas of responsibility. Proactively plans and facilitates de-scalation protocol with support of the interdisciplinary team and can resolve escalated situations within the facility.

Responsibilities

Admissions and Discharge Coordination: Manages the full admissions process, including referrals, tours, screenings, paperwork, and financial assistance; coordinates individualized discharge planning, including referrals and equipment needs.

Social Services Department Leadership: Oversees department operations, supervises social services staff, participates in facility inspections, and represents the department in leadership meetings.

Resident Advocacy and Support: Provides emotional and psychosocial support to residents and families, facilitates adjustment to facility life, communicates resident needs to staff, and fosters positive relationships and family involvement.

Compliance and Education: Maintains detailed resident records, ensures adherence to state and federal regulations, promotes a respectful and inclusive environment, and educates staff, residents, and families on rights and safety policies.

Qualifications

Licensure Requirement: Must hold a valid Minnesota license to practice social work.

Resident Care Insight: Must understand the psychosocial and emotional needs of residents.

Key Competencies: Requires strong communication, organization, management, and leadership skills.

Additional Qualifications: Must pass a criminal background check and preferably have experience in healthcare or long-term care settings.
