Overview
Physician – Geriatrics / Primary Care needed for outpatient clinic in Flint, MI. Join a program focused on caring for a geriatric population within a PACE setting, working with physicians and APPs in an outpatient clinic.
Responsibilities
Provide medical services to patients in an assigned area.
Evaluate and treat patients with appropriate medical diagnostic and treatment skills. Coordinate discharge planning with case management.
Recommend and participate in efforts to improve cost effectiveness and quality of care.
Accept referrals from other physicians. Act as admitting, attending, and/or consulting physician for patients requiring hospitalization for primary medical diagnoses, as needed.
Participate in academic programs (e.g., medical education and research) as requested.
Provide documentation meeting insurance requirements and appeal denials in a timely manner.
Participate in inpatient and outpatient consultation for specialty, which may include on-call responsibilities.
Will provide services at Ascension Living Flint, MI clinic and may round on inpatients at Ascension Living Flint long-term care facilities.
Typically see 4–6 patients per clinic day (2–3 routine assessments, plus 2 urgent visits).
Family Medicine training with Geriatric experience is preferred but newly trained physicians are considered.
Experience with simple office procedures (e.g., skin biopsy, joint injections) and goals of care/advanced care planning conversations; palliative/hospice experience is a bonus but not required.
Experience managing medically complex patients and willingness to evaluate patients in home or facility settings as needed.
Work in a team with an interdisciplinary group and adapt plans as patient needs change.
About PACE
The Program for All-Inclusive Care (PACE) coordinates preventive, primary, acute, and long-term care services so older individuals can live in the community.
Aged 55 years or older; live in a PACE provider’s service area.
Certified by the state as meeting nursing home level of care; able to live safely in the community with PACE support.
No specific financial eligibility; PACE is covered by Medicare and Medicaid for eligible individuals, with private pay options available.
Requirements
Licensure / Certification / Registration:
Physician MD/DO obtained prior to hire date. Advanced Life Support preferred (American Heart Association or American Red Cross accepted).
Education:
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
