Role Registered Nurse – Home Health

Responsibilities

Two‑week clinical orientation to learn key procedures, EMR, safety protocols and compassionate care.

Assess patients with complex health needs, develop personalized care plans and ensure the best outcomes.

Educate and guide patients and families through their healthcare journey, helping them understand their conditions and the path to better health.

Collaborate closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.

Document and track changes in patient conditions, updating care plans to reflect their progress and needs.

Qualifications

Registered Nurse (RN) with a Bachelor’s Degree in Nursing (preferred).

1+ year of clinical experience as a registered nurse (bonus points if you have experience in homecare).

Strong verbal and written communication skills – you’ll be guiding and educating patients and families.

Proficient with technology – navigating EMR systems and using digital tools to enhance patient care.

A reliable car and valid driver’s license – you’ll be traveling to patient homes, so your own transportation is key.

Physical Demands / Working Conditions

Flexibility to work both indoors and outdoors while using clinical equipment.

Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients.

You’ll be making a direct impact through client and customer contact, so your presence is invaluable.

Benefits In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $70,200 to $159,171.19. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
