About The Role
A mission-driven healthcare provider is seeking a dedicated
Clinical Transition Nurse (CTN)
to support patients and families transitioning into hospice care. This role focuses on care continuity, symptom management, and compassionate guidance during end-of-life transitions.
Key Responsibilities
Conduct comprehensive assessments of physical, emotional, and spiritual needs. Collaborate with Clinical Teams: Collaborate with clinical teams by actively sourcing hospice referrals from healthcare providers in the medical facilities. Create and manage individualized care plans focused on comfort and quality of life. Collaborate with physicians, chaplains, social workers, and other team members. Educate families about hospice services, symptom control, and care processes. Oversee transitions from pre-admission through inpatient hospice admission when applicable. Monitor patient progress and adjust care as needed. Support referral development and review insurance and consent documentation. Promote the organization’s values and compassionate service culture.
Qualifications
Graduate of an accredited nursing program; BSN preferred. Active RN license in California and current CPR certification. 1+ year hospice experience and 1–2 years in acute/home care. Must have valid driver’s license, insured vehicle, and ability to travel locally. Strong communication, problem-solving, and care coordination skills. Holistic care training is a plus.
