The Acute Care RN performs complex nursing work providing for the assessment, care and treatment of individuals who reside in a State Supported Living Center (SSLC) setting. This position is a member of the professional team and shares responsibility with other team members for the development of the individuals’ programs. Evaluates, treats and documents healthcare needs and/or issues and reports needs and/or issues to the appropriate person(s). Communicates with direct care staff, physicians/PCP, administration and other nursing staff to ensure optimal care as part of the interdisciplinary team. Notifies the supervisor and PCP of trends and concerns to ensure communication for high-quality care. Performs data entry, documentation, completes reports and maintains records. Serves as a nurse preceptor for other nurses, as needed, and may assign nursing responsibilities to the unit LVN and RN I, in accordance with the Nurse Practice Act. Participates in facility QA/QI initiatives as required. Provides treatment to HHS employees injured in course and scope of employment. Responds to emergency situations to ensure safety and care of individuals and staff. Works under general supervision of the Nurse Manager or Nursing Operations Officer with moderate latitude for initiative and independent judgment. Essential Job Functions (EJFs) / Work Expectations
Attends work regularly and may be required to work specific or rotating schedules, and overtime according to agency policy. Performs complex nursing tasks and procedures; collects specimens for laboratory examination and conducts screening tests when requested by lab personnel. Provides supervision support to the Nurse Manager with daily operations including reviews of controlled drug counts, medication counts, emergency checklist logs, refrigerator temperature logs, and glucose control monitoring. Administers medications, immunizations and treatments; observes patients for unusual symptoms and reactions; makes rounds or home visits as needed; may work at other SSLC locations as required. Monitors weight, diet, feedings, vital signs; recognizes signs of illness and develops acute care plans as needed. Evaluates nursing intervention outcomes and informs other healthcare team members; instructs direct support professionals on monitoring after treatments or procedures. Documents and reports treatments and observations in the medical record in accordance with SSLC procedures. Acts as a preceptor for other nurses and trains new nurses to work in the SSLC environment. Assists LVNs and RN Is with implementation of protocols and programs to improve health and safety; supports follow-up assessments and care planning interventions. Contributes to Infection Control, Quality Assurance and Quality Improvement initiatives; assists in implementing therapeutic nursing programs. Responds to emergencies and follows SSLC safety and infection control procedures; provides educational and safety training to SSLC staff upon request. Performs additional duties to support disaster response, continuity of operations, or other agency requirements; may involve alternate shift patterns. Knowledge, Skills And Abilities (KSAs)
Knowledge of nursing techniques and procedures; health care laws and regulations; medical diagnoses and treatments; and community health principles. Knowledge of state and federal laws and ICF-IID rules related to SSLC operations. Knowledge of nursing care standards and healthcare guidelines; ability to quickly gain necessary knowledge. Skill in patient care; strong communication skills (verbal and written). Ability to use computer software and electronic health records; ability to work independently with all levels of staff; ability to train staff. Ability to interact appropriately with individuals with intellectual/developmental disabilities, their families, and staff. Ability to make decisions and recommendations to SSLC leadership, including on-call administrators. Registrations, Licensure, Certifications
Licensed to practice as a registered nurse in the State of Texas (or in a state that recognizes reciprocity through the Nurse Licensure Compact). Initial Screening Criteria
Experience as a practicing registered nurse required; two years preferred. At least one year of RN experience in a health agency, hospital, or health-care facility preferred. Bachelor's Degree preferred. Additional Information
Applicants must pass fingerprint background check, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, and HHS List of Excluded Individuals/Entities). All State Supported Living Center Employees are subject to Random Drug Testing. Flexibility in work hours may be required; overtime and weekend/holiday work may be required.
