We are seeking a Clinical Pharmacist to join our Pharmacy team as we expand to become one of the top home infusion providers in the country. The Pharmacist will report to the Pharmacy Manager and work at a branch in your City, State.
Schedule Monday – Friday, 9:00 am – 5:30 pm

Responsibilities
Evaluates and monitors drug therapy for home care patients in collaboration with physicians, nurses, and other healthcare professionals.
Adheres to pharmacy practice standards, state and federal laws, and company policies.
Participates in quality assurance activities and accreditation standards.
Coordinates new referrals and admissions with nurses, customer service, and distribution.
Reviews, verifies, and documents new patient orders.
Assists healthcare professionals with:
Planning and implementing patient care at home.
Setting therapeutic objectives for drug therapy.
Establishing monitoring parameters for drug therapy safety and efficacy.
Preparing regular progress reports.
Conducts and documents initial and ongoing assessments of patient, drug, and disease information to identify potential drug-related problems.
Monitors drug therapy outcomes by collecting and interpreting laboratory and clinical data.
Provides consultation and answers inquiries from patients, families, physicians, and other professionals.
Maintains thorough documentation of all clinical activities.
Processes and checks all medications, supplies, and equipment before dispensing.
Maintains current pharmacotherapy knowledge through literature review and continuing education.
Compounds sterile mixtures and non-mixtures, including parenteral nutrition, antibiotics, chemotherapy, hydration, and narcotic analgesics, using aseptic technique.
Ensures compounding and cleanroom areas comply with USP 797 and USP 800 standards.
Documents all activities on appropriate logs.
Oversees Pharmacy Technician functions as required by Pharmacy Board rules.
Qualifications
Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
Registered Pharmacist License in state(s) of practice.
Minimum of two (2) years of recent experience in an acute‑care setting OR one (1) year of recent experience in home infusion therapy.
Experience with compounding infused drugs.
Proven record of evaluating and monitoring drug therapies for home care patients.
Good organizational and time‑management skills.
Participation and membership in professional societies is desirable.
Excellent verbal and written communication skills.
Flexible schedule with ability to work on‑call evenings and weekends as scheduled.
Travel: 0–25%; minimal travel may be required.
Physical Requirements Constant sitting and typing on a keyboard; occasional standing, walking, pushing/pulling, and lifting/carrying 10–30 lbs.
