As an Advanced Wound Care Nurse Practitioner in our Oncology department, you\'ll be at the forefront of healing, both physically and emotionally, for patients navigating chronic wound care. What you\'ll do
Performs comprehensive, age-appropriate patient assessments, incorporating medical history, social and cultural factors, and evaluates education needs for patients and families. Delivers skilled nursing interventions based on individualized care plans, prioritizing patient needs and proactively preventing complications. Develops and adjusts care plans in collaboration with patients, families, and the healthcare team, accounting for rehabilitation potential and discharge planning. Accurately documents patient care including assessments, interventions, education, and outcomes to ensure continuity and quality of care. Communicates effectively using structured methods to ensure clear, timely and complete information sharing among team members. Demonstrates leadership and collaboration, participates in shared decision-making, mentors staff, and takes on the charge nurse role as needed. Maintains a safe care environment, following infection control and safety protocols while managing supplies and supporting resource efficiency. Required qualifications
Possess a Master\’s Degree or higher in Nursing, or in a field deemed to be equivalent by the Oregon Board of Nursing. Have satisfactorily completed a Nurse Practitioner Program recognized by the Oregon Board of Nursing. Hold a current, unencumbered Registered Nurse license in the State of Oregon. Hold valid, unencumbered Nurse Practitioner national certification issued by ANCC or NCC, or an equivalent Medicare/Medicaid approved certifying body or be actively seeking certification and obtain the same in the specialty area requested within 24 months of privileges being granted at Salem Hospital. Hold DEA certification. At least 12 months relevant clinical experience in the expanded specialty area during the past three years, required. Additional information
Certified Wound Care Nurse Advanced Cardio Life Support - within 6 months of hire If the Nurse Practitioner does not have the relevant experience and cannot otherwise document current competency, a monitoring plan may be required. Knowledge of Lean methodology preferred Total rewards package
