Graduate of an accredited university with an MA or MS in communication disorders that is recognized by ASHA and the state board. Holds current license and/or registration in the state (as applicable). Successful completion of CFY. Responsibilities

Provide comprehensive speech therapy evaluations as per MD orders. Document findings on the standardized evaluation format neatly, accurately and adhering to guidelines. Develop and implement a comprehensive treatment plan with long- and short-term goals, frequency, duration, modalities, and interventions. Obtain MD orders for evaluations, treatments, recertifications and discharges. Provide comprehensive treatment to patients utilizing appropriate modalities and modify treatment as indicated while adhering to precautions. Document patient progress in approved formats accurately and timely. Maintain monthly updated treatment plans in standardized format in a timely manner. Document patient discharge and in-service as indicated; document caregiver or discharge in-services as needed. Complete all required documentation for the facility and assist with MDS-related information as deemed necessary. Attend facility and rehab meetings and participate in committee activities as indicated. Act as a rehab delegate to the interdisciplinary team and participate in patient scheduling. Perform screens as indicated and complete billing timely and accurately for services rendered. Cover other therapists’ caseload during absences as necessary. Supervise junior staff or students in compliance with state ST Board and federal regulations; co-sign notes as required. Maintain professional relationships with co-workers, staff, patients and families; provide in-service education as needed. Comply with facility policies, safety guidelines and patient confidentiality/HIPAA. Report to work on time, adhere to scheduled hours and maintain a professional image; meet productivity requirements and submit reports timely.
