Job Overview The Pharmacist is responsible for providing medication management, clinical support, and consultative services in a complex pharmacy operation. The position partners with key stakeholders to drive patient and family education, medication safety, quality assurance, customer service, process improvement, and regulatory compliance. The Pharmacist may also participate in education of pharmacy students and residents.
Location UMMC East Bank Hospital, Minneapolis MN
Hours 72 hours bi-weekly
Schedule Day/evening, every third weekend
Responsibilities
Provide clinical, formulary, and pharmacotherapy expertise and act as a resource to students, staff, and interdisciplinary members of the care team.
Provide clinical pharmacy services to all patients in assigned area using Standard Operating Procedures (SOP). This includes dispensing medication, completing review for all patients, monitoring, adjusting patient therapies, and documenting changes according to department policies and pharmacy regulations.
Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness, and compliance with any other formulary or departmental initiatives as appropriate.
Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures, and other extemporaneous orders as appropriate.
Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences, and actively participating in department clinical meetings and activities.
Participate in regulatory, quality assurance and process improvement activities as required.
Meet all regulatory requirements.
Use available information technology and other resources to support day-to-day work operations.
Required Qualifications
Minnesota Pharmacist License
Preferred Qualifications
Doctor of Pharmacy degree
1 year pharmacist experience in a complex healthcare environment
Completion of an ASHP-accredited PGY1 residency program
