Plan and provide speech and language rehabilitation services to patients in their homes. Work with patients, families, and professional staff to establish rehabilitation goals and activities. Evaluate patient needs and perform clinical tests to assess speech and language disorders. Develop plans of care with restorative and maintenance goals. Instruct patients and families in the use of assistive modalities and speech language procedures. Maintain patient records, including progress reports and discharge summaries. Communicate with the nursing team, coordinate care, and re-evaluate patients as needed. Qualifications
Master’s Degree in Speech Language Pathology. Minimum of one (1) year of experience in a health care setting. Knowledge of current standards, procedures, and techniques of speech language pathology (home‑care experience desirable). Current licensed in New York State as a Speech Language Pathologist and clinical completion certification from the American Speech‑and‑Hearing Association. Benefits
