The Home Health Physical Therapist - PRN is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation. S/he promotes the Compassus philosophy using the 6 Pillars of Success as the foundation, upholds the Code of Ethical Conduct, and fosters positive working relationships within the organization and with external stakeholders. The licensed professional provides coordinated care to patients of all ages, planning, implementing, and evaluating patient care plans to restore or maintain well-being. Responsibilities include conducting assessments, collaborating with the interdisciplinary team, preparing and revising care plans, and instructing patients and families on transfer techniques and equipment use. Education and Experience:
Bachelor of Science in Physical Therapy required; two years of experience in an acute care or rehabilitation setting preferred. Skills:
Strong communication, mathematical, and clinical skills; ability to articulate and embrace hospice philosophy; proficiency in English. Certifications and Licenses:
Active and unencumbered Physical Therapist license in applicable state(s); current CPR certification. Physical Demands and Work Environment:
Must perform essential functions including standing, walking, lifting up to 50 pounds, and working in a healthcare setting with exposure to bodily fluids and infectious diseases.
