Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a non-profit that delivers medical, social, and behavioral services to the frail elderly in the North County of San Diego. This PACE center is entrepreneurial and focuses on innovations and bringing new processes to the traditional PACE environment. We\'re looking for passionate healthcare providers who want to be part of a team dedicated to enabling seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care. Position Summary The West PACE Physical Therapist coordinates and provides restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with the PACE Physician, rehabilitation staff and the interdisciplinary team (IDT) to maximize participant independence and safety and to develop or restore function. The Physical Therapist collaborates with the Occupational Therapist to operate West PACE rehabilitation department and works with colleagues and stakeholders to promote the West PACE values, team culture and mission. Essential Functions Conduct Physical Therapy assessments to determine care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans. Deliver and document Physical Therapy interventions in accordance with physician orders and/or care plans, including active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water, light or electricity. Supervises the Therapy Assistants including providing training and guidance needed to implement Functional Maintenance Programs (FMP). Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment. Order, track and manage maintenance for equipment. Remain alert to input from other team members, participants, and caregivers and update the IDT promptly of any changes in condition or functional status. Work with IDT members to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home). In coordination with the Marketing Team, support enrollment of prospective participants into the program. Participate in end-of-life care coordination and support. Complete all required documentation in a timely and accurate manner. Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families. Follow West PACE policies and procedures and participate in required Quality Improvement activities, staff training and meetings. Other duties as assigned. Education And Certification Valid Physical Therapy license in the state of California, or proof of license. Master\'s degree in Physical Therapy; or Master\'s degree in a PT-related field in addition to a bachelor\'s degree in Physical Therapy; or equivalent. Current Basic Life Support (BLS) / First Aid Certification required. Current driver\'s license and proof of auto insurance required. Skills And Experience Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population. Knowledge and skills to treat frail, elderly participants with complex needs. Highly motivated, self-directed, able to work in a quickly changing environment and make sound decisions in emergencies. Excellent clinical, organizational and communication skills with seniors, their families and IDT members. Experience and competency working with people from diverse backgrounds. Working Conditions And Physical Demands The physical demands and work environment characteristics are representative of those required to perform the essential functions of this role. Work Environment: Interdisciplinary team model in an Adult Day Health Center, primary care clinic and participant home environment with moderate noise levels and controlled temperatures. Physical Requirements: Requires corrected vision and hearing; ability to communicate with staff, vendors and clients; manual dexterity and eye-hand coordination; lifting up to 35 lbs. and pushing/pulling 50 lbs with proper technique and accommodations if needed. Vision, Hearing and Communication: Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Pressure Factor: May involve stressful conditions, noisy and crowded environments, fluctuating temperatures and potential for verbal aggression. Environmental Conditions: Possible exposure to moving equipment, substances, bodily fluids, and other conditions common in healthcare; may include unpleasant odors. Medical Clearance Employees must have medical clearance for communicable diseases and up-to-date immunizations before direct participant contact.
