Job Summary:
We are seeking a compassionate and skilled Wound Care/Infection Preventionist Nurse to join our team. The Wound Care/Infection Prevention Nurse will assess, plan, and implement comprehensive wound care and infection surveillance for residents. This role requires strong clinical skills, knowledge of wound care management, infection prevention, and a commitment to high-quality patient care.
Work Environment:
This position operates in a clinical healthcare setting.
Responsibilities
Assessment and Planning: Conduct thorough assessments of residents’ wounds and infection prevention needs. Develop and implement individualized care plans based on assessment findings and best practices.
Wound Treatment: Administer wound care treatments, including dressing changes, debridement, and application of topical agents. Ensure proper techniques and infection control practices are followed.
Infection Preventionist: Lead surveillance activities, document incidents and corrective actions, and report findings to the facility’s Quality Assessment and Assurance Committee.
Collaboration: Work with physicians, nurses, and other professionals to coordinate care and develop treatment plans. Educate nursing staff on wound care and infection prevention.
Monitoring and Documentation: Monitor and document wound healing and infection prevention progress, changes in condition, and responses to treatments. Maintain accurate records per facility policies and regulatory requirements.
Patient and Family Education: Educate residents and families on wound care, prevention strategies, and treatment adherence. Address questions and concerns.
Quality Improvement: Participate in quality improvement initiatives related to wound care and infection prevention.
Emergency Response: Respond to urgent wound care/infection prevention needs and manage complex cases in collaboration with the care team.
Qualifications
Education: Registered Nurse (RN) with a valid state nursing license. Wound Care Certification (WCC) and Infection Preventionist (IP) certification required. If not certified, must obtain certification within 30 days of hire.
Experience: Minimum of 2 years in wound care, preferably in a skilled nursing facility or similar setting.
Skills
Strong clinical assessment and critical thinking skills.
Proficiency in wound care management and treatment techniques.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a team environment.
Strong organizational and time-management skills.
