Overview
Patient Services Coordinator-LPN role at CenterWell Home Health. Responsible for scheduling visits, coordinating with field staff, patients, physicians, and maintaining care continuity. Provides day-to-day office and staff management support. Responsibilities
Manages schedules for all patients; edits schedules when agents are unavailable, reassigning patients in a timely manner and updating worker availability in the system. Initiates infection control forms and submits the completed Employee Infection Report to the HR/worker console for uploading. Acts as back-up during lunch hours and peak times; handles calls from field staff and assists with weekly case conferences; refers clinical questions to Branch Director as needed. Maintains the client hospitalization log, records coordination notes, and sends electronic logs to office, field, and sales staff. Completes requested schedules and ensures staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is planned with the injection visit. Completes added-on and applicable orders: schedules discharge visits/OASIS collection or recert visits after case conferences; schedules TIF OASIS collection visits and deletes remaining schedules; reschedules declined or missed visits; processes reassigned and rescheduled visits; ensures supervisory visits are scheduled. Runs scheduling reports (e.g., Agent Summary Report, Missed Visits Done on Paper Report). Prepares weekly agent schedules and conducts an initial productivity/geographic review before branch director approval and distribution. Verifies visit notes in scheduling console as needed. Assists with internal transfer of patients between branch offices. If clinical, receives lab reports, assesses normality, forwards copies to doctors and Case Manager, and routes to Medical Records; initiates Employee/Patient Infection Reports as necessary. If clinical, may be required to perform patient visits and participate in on-call rotation. Qualifications
Be a Licensed Professional Nurse (LN) or Licensed Vocational Nurse (LVN) licensed in the state of practice. Have at least 1 year of home health experience. Prior packet review/QI experience preferred. Coding certification is preferred. Must possess a valid state driver’s license and automobile liability insurance; must be licensed in the state of employment if applicable. Excellent communication skills, ability to interact with a diverse group, strong organizational skills, and ability to manage multiple priorities. Job Details
