Hartford HealthCare’s Institute of Living, a leading mental health center, offers comprehensive care, research, and education for behavioral, psychiatric, and addiction disorders across all ages, with specialized programs in Anxiety, Eating Disorders, and Geriatric Services. Job Summary Collaborate with interdisciplinary teams to organize, prepare, and deliver evidence-based clinical services across the care continuum, maintaining ethical standards and professional practice. Key Responsibilities Conduct comprehensive bio-psychosocial assessments and develop care plans. Coordinate with the team to assess care levels and develop discharge plans, utilizing appropriate therapeutic methods. Monitor and revise interventions based on patient progress. Provide education to patients and families about health management and treatment adherence. Deliver direct patient care, including case management, crisis intervention, counseling, and discharge planning. Refer patients to appropriate resources and ensure timely documentation. Independently perform clinical social work interventions across patient populations. Participate in team meetings, supervision, and staff development; supervise social work students. Support hospital and unit programming aligned with clinical needs. Contribute to a positive patient experience and ensure resource utilization efficiency. Qualifications Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or related field. Experience in a healthcare setting. Licenses: LCSW, LPC, or LMFT. Valid Driver’s License and proficiency in computer use. Strong decision-making, communication, and cultural competence skills. Benefits Medical and dental coverage. 401(k) with employer match.
