Scope of work
Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes. Qualifications
Required Bachelor\'s Degree Nursing Preferred Master\'s Degree Nursing 5 years of relevant experience (previous nursing or CDE clinical experience) Required Prior teaching with patients of all ages Preferred LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 120 Days required or CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 120 Days required Benefits and compensation
