Nurse Practitioner Primary Care (Full-Time)
We are seeking a compassionate and experienced Nurse Practitioner to provide primary and preventive care for adult patients residing in long-term care settings. You will be part of an interdisciplinary care team focused on improving the quality of life and health outcomes for patients in skilled nursing environments. About The Opportunity We are seeking a compassionate and experienced Nurse Practitioner to provide primary and preventive care for adult patients residing in long-term care settings. You will be part of an interdisciplinary care team focused on improving the quality of life and health outcomes for patients in skilled nursing environments. Key Responsibilities Deliver primary and preventive care services to patients in skilled nursing settings Manage acute changes in patient conditions Conduct chronic care management and advanced care planning Prescribe medications, including controlled substances in compliance with state law Order and interpret diagnostic tests and treatments Collaborate with nursing staff and other healthcare professionals to adjust care plans as needed Communicate clinical updates and recommendations clearly and regularly Coordinate with primary care physicians to ensure continuity of care Required Qualifications Active and unrestricted NP license in North Carolina Board Certification (NP) DEA registration for prescribing controlled substances Experience in long-term care, adult, family, or geriatric medicine Proficiency with Electronic Medical Records (EMR) Ability to manage patients with advanced illness and chronic conditions Strong communication and teamwork skills to function in an interdisciplinary environment Education Requirements Master's Degree in Adult-Gerontology Nurse Practitioner specialization Bachelor's Degree in Nursing (RN to BSN) High School Diploma Preferred Skills Familiarity with long-term care protocols Background in psychiatric care (preferred but not required) Benefits Overview Competitive salary with Bonus Pay eligibility $5,000 Sign-On Bonus Malpractice Insurance (provided) Health, Dental, and Vision Insurance 401(k) with company matching Paid Time Off (PTO) Reimbursement for recertification and continuing education We are an equal opportunities employer and welcome applications from all qualified candidates.
