$43.30/hr - $64.95/hr What You’ll Do
As a Physical Therapist in our Home Health division, you’ll:
Coordinate and deliver medically prescribed physical therapy services with a focus on quality, continuity, and cost-effectiveness. Conduct comprehensive evaluations and develop personalized care plans based on musculoskeletal, neuromuscular, cardiopulmonary, and integumentary assessments. Monitor patient progress, adjust treatment plans, and communicate updates to physicians and leadership. Advocate for patients by managing their medical conditions, medications, and functional goals. Supervise Physical Therapist Assistants (PTAs) and collaborate with Home Health Aides to maximize patient outcomes. Educate patients and caregivers on treatment techniques and adaptive equipment. Participate in chart audits, quality improvement initiatives, and discharge planning. Mentor new employees and serve as a clinical instructor for students.
What You’ll Need
Licensure: Physical Therapist license (Wisconsin) Certifications: BLS for Healthcare Providers (AHA) – must be obtained within 6 months Education: Bachelor’s Degree in Physical Therapy Experience: Minimum 3 years in acute hospital, home health, or rehab setting Skills: Strong clinical judgment, communication, organization, and decision-making abilities Tech: Basic proficiency in Microsoft Office (Word, Excel, PowerPoint) Transportation: Valid driver’s license and insured, functional vehicle
