Overview

HealthCare Access Maryland (HCAM) is a nonprofit organization dedicated to making Maryland healthier by connecting residents to health insurance coverage and providing comprehensive care coordination. HCAM assists thousands of individuals each year with guidance, education, and advocacy to ensure access to health services. The Community Health Advocate (CHA) will use trauma-informed strategies to outreach pregnant and postpartum women who are difficult to locate or hesitant to engage with care coordinators at first outreach. The CHA will be embedded in Judy P. Hoyer Center Early Learning Hubs (Judy Centers) to locate pregnant women and reconnect them to HCAM’s Care Coordination Program and to connect them with services for parents, children, and families offered by the Judy Center. The CHA will work to build trust with high-risk, hard-to-reach pregnant women in the targeted region and connect them to quality client services. Responsibilities

Administration: Participate in professional development opportunities in client engagement, motivational interviewing, and trauma-informed outreach; participate in local/state meetings and community events; communicate consistently with supervisor. Community Partnerships/Outreach: Partner with Judy Center staff, host/cohost events, develop referral systems between Judy Center and HCAM to reconnect pregnant women to prenatal services. Case Management: Assess, triage, and refer pregnant/postpartum women and infants to appropriate programs; help remove barriers to healthcare access; contact new cases within 15 days; locate clients; assess needs and coordinate referrals to OB, MCO, pediatric, primary, dental, and other services; maintain performance metrics (e.g., timely case closures). Documentation: Document outreach activities in the electronic medical record (eClinicalWorks); maintain precise case notes and update outreach trackers daily. Human Resources: Submit timesheets timely and complete required trainings. Additional duties as assigned. Qualifications

High School Diploma or GED; Bachelor’s Degree in a health-related area preferred. Certification in Community Health Work is a bonus. At least two years of home/community outreach experience in maternal and child health in Baltimore City. Bilingual in Spanish is preferred but not required. Knowledge of community outreach and Medicaid system navigation; experience with HealthChoice Medicaid recipients and Managed Care Organizations. Strong organizational, problem-solving, and communication skills; proficiency with MS Word, Excel, Outlook, and SharePoint; ability to work independently and in a team. Compensation

Base pay range: $24.00/hr – $24.00/hr. This range is provided by HCAM; actual pay will be based on skills and experience.
