Pharmacists provide pharmacy services with compassion, excellence, and efficiency. CHRISTUS Santa Rosa Hospital - New Braunfels (CSRH-NB), located in downtown New Braunfels, is a full-service facility offering comprehensive services to meet patient needs.

Responsibilities

Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.

Prepares and dispenses drug orders per physician request according to established policies, procedures, and protocols.

Interprets drug orders (verbal and written) and transcribes/verifies into the computerized patient medication record; maintains accurate patient drug records.

Compounds and dispenses pharmaceuticals including non-sterile and sterile products, chemotherapy and parenteral nutrition products accurately.

Issues and records controlled substances per legal and institutional policies.

Provides services efficiently and in a timely fashion; maintains current pharmacist license and CE records.

Attends staff meetings and completes competency/skills assessments as required.

Supervises and directs pharmacy support personnel and verifies the daily activities of pharmacy technicians.

Ambulatory care Pharmacist: Provides ambulatory pharmaceutical care services utilizing approved protocols and/or collaborative agreements, including managing patient drug therapy, anticoagulation management, patient education, and preparation/oversight of medications in clinical areas such as anticoagulation clinics, transition of care, and discharge medication reconciliation.

Clinical duties include ensuring safe, appropriate, cost-effective drug therapies; monitoring for contraindications and interactions; assisting with pharmacokinetics and dosing; interpreting patient records; reporting adverse drug reactions; maintaining formulary and providing clinical consultations and drug information.

Participates in quality improvement, antibiotic stewardship, opioid stewardship, shortage management, and interdisciplinary rounds when assigned.

Education/Skills

Doctor of Pharmacy or BS Pharmacy degree required.

Experience

Hospital experience preferred.

Licenses, Registrations, or Certifications

Pharmacy state licensure required within 60 days of start date.

BLS or ACLS required; PALS for pediatric practice.

In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all associates are required to obtain the required certifications for their position within the designated time frame.
