Job Description The LPN/LVN administers nursing care according to the plan of care to terminally ill patients admitted to the VITAS program, in accordance with the Nurse Practice Act in the State where the VITAS program is located.
Responsibilities
Provide direct skilled bedside nursing care.
Evaluate patient/family needs and assist in the preparation and execution of the interdisciplinary care plan.
Perform acts in the care of the terminally ill under supervision, including administration of treatments and medications.
Evaluate treatment effectiveness and monitor patient reactions.
Report significant findings or changes to the healthcare team and document appropriately.
Maintain a clean and safe environment for patients and families.
Maintain accurate documentation, including clinical notes.
Utilize supportive and technical nursing skills for medical orders, treatments, and monitoring.
Assist in designing and delivering the interdisciplinary care plan.
Establish therapeutic relationships with patients and families, encouraging participation in care.
Initiate and perform nursing protocols based on patient needs.
Participate in patient and family teaching about illness, treatment, and self-care.
Engage in professional growth activities and community education.
Participate in on-call rotations and work flexible schedules, including weekends and shifts.
Requirements
Graduate from an approved Practical/Vocational nursing program.
Current nursing license in the state where the program is located.
At least one year of satisfactory nursing experience, preferably in a hospital setting.
Must have reliable transportation with appropriate insurance and license.
Physical and Vision Requirements Assist in lifting patients, stand or walk for extended periods, and perform physical tasks. Visual abilities include close and distance vision and ability to focus.
