Atrium Health Wake Forest Baptist The Population Health Pharmacist embedded in Downtown Health Plaza Clinic will be accountable for carrying out medication-related strategies designed to optimize patient care and outcomes in the WFBH value-based care (VBC) population specific to Managed Medicaid. Responsibilities include providing patient-centered care, aiding in the closure of medication-related care gaps, ensuring access to medications, and precepting residents and students. The pharmacist will work collaboratively with members of the multidisciplinary population health team, clinic staff, and other areas as needed. Position Highlights Clinical Pharmacist, Population Health, Downtown Health Plaza Monday-Friday 8am-430pm Winston Salem, Downtown Health Plaza Clinic Education/Experience/Certifications To Qualify Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) Current state pharmacist license in good standing through North Carolina Board of Pharmacy Completion of an ASHP-accredited PGY1 residency or equivalent experience Major Responsibilities Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. Uses effective patient education techniques to provide counseling to patients and caregivers. Designs, recommends, monitors, and evaluates patient-specific pharmacotherapeutic regimens.
