Erie Family Health Centers is looking for a valuable addition to our Health Educator team. The Health Educator works independently and as part of a team to support internal and external community health programs. Main Duties & Responsibilities Conduct educational appointments with patients, prepare for and facilitate group sessions, provide patient follow-up, and present educational workshops that focus on nutrition, diabetes, disease prevention, and physical activity. Provide training and assistance for staff regarding health education issues. Act as a member of a patient care team with clinical staff and other health care providers. Communicate effectively with patients, health care providers, and community members. Qualifications Education Associate’s degree in Community Health or Health Education (or equivalent education and directly related experience). Skills And Knowledge One or more years working in a community health setting preferred. Experience conducting individual or group education presentations. Strong interpersonal and oral communication skills. Ability to communicate effectively in both English and Spanish is required (may be required to assist or communicate with non-English/Spanish speakers). Demonstrated cultural competency to work in a low income, minority environment preferred.
