Overview

This is a hybrid position located in St. Petersburg, FL. The requirement is to come into the office one week and work from home the next, rotating weekly between in-office and remote work. Responsibilities

Prevents, detects and resolves medication-related problems and provides evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary team. Identify and resolve medication therapy problems, including inappropriate indications, dosing, routes, therapy duration, interactions, allergies, and documentation issues. Individualize medication regimens considering pharmacodynamics and pharmacokinetics; contribute to delivering pharmaceutical care within areas of direct responsibility. Provide clinical consultation to dialysis clinic staff, physicians, patients and internal personnel regarding medications, interactions, side effects, dosing, and storage. Develop clinical logic and algorithms to support clinical outreach programs; collaborate with the Director of Clinical Services and Business Analyst to design reports and systems for clinical pharmacy services. Facilitate resolution of barriers to filling, adherence, and appropriate use of prescribed medications. Stay current with clinical literature related to CKD and related comorbidities; maintain competency to provide care to all ages and types of patients in dialysis clinics. Ensure prescription accuracy, resolve inconsistencies, and follow up with appropriate personnel to ensure problems are resolved. Participate in pharmacy CQI processes and report errors as appropriate. Maintain expertise in pharmacy software, CRM tools, and electronic health records; navigate systems to locate patient data and document appropriately. Build productive working relationships within the pharmacy, the FMCNA organization, and external partners; provide excellent service regarding clinical information requests. Assist with various tasks as directed; escalate issues to supervision when needed; refer to senior staff for higher-level problems as appropriate. Review and comply with Code of Business Conduct and applicable laws and policies; assist with assigned projects; obtain state licensures as needed to support FreseniusRx initiatives. Other duties as assigned. Qualifications

Doctor of Pharmacy (Pharm.D.) degree from an accredited school; licensed in the appropriate state. ASHP-accredited PGY1 Pharmacy Practice Residency completed; PGY2 preferred. Completion of an ASHP-accredited PGY-1 residency and 2+ years of clinical experience. Strong clinical background in chronic kidney disease and end-stage renal disease preferred. Pharmacokinetic monitoring skills; MTM and patient interviewing skills. Excellent written and verbal communication, organizational, and analytical skills. Proficiency with Microsoft Office and clinical systems; ability to learn new systems quickly. Knowledge of current pharmacy law; ongoing continuing education in CKD/ESRD. Education

Doctor of Pharmacy (Pharm.D.) degree from an accredited college or school of pharmacy. Completed ASHP-accredited PGY1 Pharmacy Practice Residency; PGY2 preferred. Appropriate state licensure. Experience

2+ years of clinical experience; completion of an ASHP-accredited PGY-1 clinical pharmacy residency. Strong background in CKD/ESRD preferred; pharmacokinetic monitoring and MTM experience. Physical Demands and Working Conditions

Must be able to sit, stand, and work at a computer for long periods; able to answer and communicate via telephone; lift up to 10 pounds. Travel to meetings or training venues as needed. Supervision
