AMBULATORY CARE- Wellness Program Charlotte, NC - Hybrid Position Monday-Friday (8am-5pm) Novant Health is seeking a Clinical Pharmacist to join our ambulatory pharmacy team, delivering optimal medication management across all dimensions of care, every time. This role will focus on providing clinical and operational support for our Healthy Lifestyles Program. Potential relocation assistance for qualified applicants & locations! Will need to come on-site as needed.

Responsibilities

Partner with NHMG providers and clinical staff to provide remarkable patient care. Conduct medication therapy management and patient counseling. Promote high quality, evidence-based, cost-effective pharmaceutical care. Participate in pharmacy and NHMG committees and shared governance. Demonstrate understanding of quality improvement principles and contribute to process improvement projects. Support the educational and professional development of the department and system. Communicate effectively in writing and orally; demonstrate proficiency with equipment and software. Onboarding will require in-person relationship building with clinics; additional onsite presence may be required to maintain relationships and build referral volume.

Qualifications

Education: Bachelor’s degree in Pharmacy (4 Year) or PharmD; FPGEC Certification required; PharmD preferred. Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a licensed pharmacist in health system pharmacy; oncology experience preferred for oncology roles, required. For Infectious Diseases/Antimicrobial Stewardship roles: ASHP-accredited PGY2 Infectious Disease residency OR minimum of 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required. Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date; Board of Pharmacy Specialties Certification or other approved certification must be obtained and maintained within 2 years of start date as applicable to the role. Additional Skills: Ability to drive/travel to multiple facilities/locations; ability to work as part of a high-performing, change-ready and resilient team.
