The Hospice Supervisor coordinates the integration of Care/Case Management functions to provide efficient and effective outcome-based, patient-centered care. You will provide oversight of Hospice staff, including leading, educating, and assessing performance of staff.

What You Can Expect

Full Time Mon-Fri from 8:30am-5:00pm (rotates through administrative call)

Be a part of a collaborative and supportive team

Develop and mentor others

Serve as expert and resource to staff

Work collaboratively to improve outcomes

Qualifications

Education: Bachelor's Degree in Nursing

Experience: 3 years of relevant experience, preferably in Hospice setting

Licenses/Certifications: Current RN licensure in State of Practice; current Basic Life Support (BLS) Provider Certification through American Heart Association or American Red Cross
