Field RN, Care Manager, Home Health, East Portland – join Adventist Health Portland. This role plans, organizes and directs home health services for patients in their homes, managing a patient caseload and collaborating with core disciplines to meet care goals. Job Summary

Plans, organizes and directs home care services to meet the needs of patients in their homes. Manages a patient caseload in an effective and efficient manner. Provides patient care independently, but collaborates with other core disciplines in accordance with the patient’s plan of care to meet care goals. Functions as a resource to patients, family members, and other care team members. Job Responsibilities

Complete initial and ongoing comprehensive assessments of the patient’s needs at appropriate time points. Perform nursing assessments and discuss and teach pertinent aspects of nursing care and patient rights and responsibilities. Identify and report problems, abnormal findings, or nursing diagnoses requiring follow-up. Implement plan of care in accordance with identified needs. Partner with the patient to clarify mutually agreed-upon goals and incorporate these goals into the plan of care. Provide direct and/or indirect skilled, safe, quality care. Coordinate patient care with other disciplines and integrate input from disciplines. Perform ongoing nursing assessments and adjust plans of care as needed. Accurately complete and document all pertinent information in accordance with policy, documenting patient care delivery, patient status, response to treatment, and interventions. Ensure timely documentation and updates to nursing plans and medication profiles when changes occur. Counsel and educate the patient and family to meet healthcare and nursing needs. Provide learning opportunities for patient and caregiver, including written materials as appropriate. Initiate preventative and rehabilitative nursing procedures when indicated. Comply with applicable legal requirements, standards, policies and procedures (e.g., code of conduct, HIPAA, documentation standards). Demonstrate professional conduct and commitment to Adventist Health’s mission and values. On-call duties may be assigned based on location and agency needs. Perform other job-related duties as assigned. Qualifications

Education and Work Experience

Bachelor\'s Degree in Nursing (BSN): Preferred Acute care facility or home care experience: Preferred Licenses/Certifications

Registered Nurse (RN) licensure in the state of practice: Required Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS or HS-BLS or RQIBLS) certification: Required Valid Driver\'s License (DL) and must be at least 21 years of age or older: Required Current automobile insurance and a reliable automobile: Required Basic Life Support (BLS or HS-BLS or RQI BLS) certification: Required Essential Functions

Implement and adjust the plan of care; coordinate with other disciplines and incorporate input; provide safe, quality care. Document all aspects of care accurately and timely; maintain updated nursing plans and medication profiles. Counsel and educate patients and families; provide learning opportunities and preventative/rehabilitative procedures as appropriate. Ensure compliance with legal and organizational standards (HIPAA, code of conduct, documentation); uphold Adventist Health values; on-call duties may be assigned.
