Position Summary
The nurse navigator provides individualized assistance to specific patients and their families in overcoming barriers to care, facilitating timely access to care, and providing education and support. The patients' trajectory through diagnosis, treatment, and after care will be seamless and coordinated in collaboration with the multidisciplinary team. Additional patient navigational needs may be assigned to this Navigator.
Responsibilities
Patient assessment and identification of patient specific needs: Identify barriers and have the knowledge to overcome those barriers, locate resources, coordinate services, provide education on treatment side effects and develop structured means for patients to report; refer patients to integrative therapies and support groups. Communication: Serve as primary contact and advocate for patient; build and maintain trust of patient and family; provide emotional support; navigate the health care system using knowledge of the intricacies of each system and serving as the patient advocate assuring timely access; serve as point person for physicians. Resource during treatment: Maintain/utilize a comprehensive database of resources, both locally, regionally, and nationally, for patients and families including financial, psychosocial, education, emotional, transportation, and cultural. Collaboration with medical team: Create and maintain open lines of communication with physicians and their support staff; establish referral pathways thereby decreasing treatment delays: work collaboratively with the health care team; maintain a network of specialists and support best practices in care delivery. Quality assurance: Maintain accurate and complete documentation; ongoing assessments of role in decreasing disparities in care and reduction in outmigration of patients; submit data to supervisor for quality monitoring and financial accounting.
Minimum Requirements
4 year college degree is required. BSN, MSN preferred Current North Carolina nursing license is required. 3-5 years clinical experience for patient focus is required. 3-5 years in a hospital setting is required. Certification for patient focus preferred or within 2 years of employment.
