Responsibilities
Develops and maintains the social service program, drafting supporting policies, procedures, and guidelines; maintains knowledge of referral resources across the service area.
Makes home visits to assess patients’ psychosocial status; communicates with physicians to report progress, needs, concerns, and potential and actual problems; maintains accurate records including clinical and progress notes per policy.
Collaborates with the Lumiere team to develop treatment plans, including discharge planning, based on psychosocial assessment and social work interventions.
Develops relationships with leaders in community resource groups and represents the agency at meetings and committees addressing social needs (e.g., Council on Aging, Developmental Services).
Acts as a resource for other health care team members, providing consultation on social and emotional factors related to health problems.
Maintains knowledge of Medicare, Medicaid, and other third-party reimbursement sources.
Participates in planning and implementing quality improvement activities.
Provides positive client relations with patients, families, physicians, colleagues, and partner agencies.
Respond to emails daily and complete assignments within 48 hours.
Adheres to company policies; may perform other related duties as assigned.
Required Skills/Abilities
Physical, sensory, and cognitive abilities sufficient to perform essential functions.
Good interpersonal and written and verbal communication skills.
Education and Experience
Master’s degree in social work.
At least one year of social work experience in a health care setting; home care experience preferred.
Valid driver’s license and own transportation to make home visits.
Licensed Clinical Social Worker in the State of Illinois.
Physical Requirements
Prolonged periods of sitting at a desk and working on a computer, with standing, bending and moving as needed.
Must be able to lift up to 15 pounds at times.
Ability to complete assignments in varied weather and environments in patient homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
May be exposed to patients with infectious diseases.
