Job Overview

At NorthBay Health the Inpatient Case Manager assesses, plans, implements, monitors and evaluates options and services to affect an appropriate, individualized plan for hospital patient care across the continuum of care. The case manager follows assigned patients working with team members and Social Services to prioritize and strategize each day’s activities. Using independent judgment and discretion, the patient must be assessed and reassessed as an ongoing process to address the patient’s broad spectrum of needs. Responsibilities

Assess, plan, implement, monitor and evaluate patient care options and services. Prioritize and strategize daily activities with interdisciplinary team and social services. Continuously assess and reassess patient needs and adjust plans. Utilize clinical knowledge and positive communication skills. Ensure adherence to department and system policies and quality assurance guidelines. Qualifications

Education:

Graduate from an accredited professional nursing school; BSN required. Licensure/Certification:

Current California RN license; Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred. Experience:

Minimum 2 years acute clinical nursing, case management experience strongly preferred, experience can substitute ME in Nursing with emphasis in Case Management. Skills:

Basic PC knowledge and software; ability to enter and retrieve data; knowledge of MCG, Cerner, Medicare rules, Conditions of Participation, Joint Commission, DHHS, patient safety, health care standards, levels of care, medical necessity guidelines, quality assurance. Interpersonal Skills:

FTE: 0.9 (Full-Time). Schedule: 72 hours per pay period. Shift Length: 8-hour shifts. Days: Weekdays and every other weekend (Saturday and/or Sunday, based on business needs). Compensation

Hourly Salary Range: Min $78.00 – Max $94.04 (based on years of Case Management experience). About Us
