Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program.
The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. We have an exciting opportunity to join our team as an Assistant, Patient Navigator (Bilingual Proficiency in Spanish is Preferred).
In this role, the successful candidate will be responsible for providing culturally competent anonymous and confidential HIV counseling, testing and referrals. The Patient Navigator Assistant will provide sensitive harm reduction counseling, HIV prevention education, and referrals to clients accessing HIV testing.
Job Responsibilities:
Performs other duties as assigned in accordance with the mission, goals, and values of the Family Health Centers at NYU Langone Complies with all organizational policies and procedures. Patient Navigator is expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment and demonstrates confidence in decision-making and in building relationships. Provides critical feedback to other members of the health care team based on observations in the field and outreach as necessary Interacts with patients/staff in a courteous manner and communicates with other departments to facilitate patient flow and/or resolve patient issues. Coordinates the maintenance of and proper stocking of community safer sex/information and distribution. Contributes and leads prevention education campaigns and resource development including dissemination. Identifies and responds to emerging community issues in HIV prevention through the development of innovative outreach education, health promotion and community programs Coordinates with representatives of other related agencies to communicate information and develop partnership opportunities for outreach. Minimum Qualifications:
To qualify you must have a High School Diploma Required. Some college preferred. Past experience and employment working with HIV care and/or prevention. Experience working with LGBTQ communities. Experience with effective behavioral interventions for HIV prevention and able to conduct effective outreach to promote prevention within the LGBTQ communities. Preferred Qualifications:
