Job Overview Our home health and hospice agencies serve communities across California, Hawaii, Oregon, and Washington, providing personalized in-home care and earning recognition such as the Home Care Elite awards. Key Responsibilities Manage patient discharges, develop post-hospital care plans, and coordinate with healthcare teams and community resources. Supervise staff activities, assist in recruitment, training, and performance evaluations. Ensure compliance with standards of care, compile statistics, and participate in marketing and strategic planning. Qualifications Bachelor’s Degree in nursing or related field (required); Master’s Degree (preferred). Minimum five years of home care nursing experience and one year of leadership experience (preferred). RN or PT licensure in the state of practice (required). Valid driver’s license, at least 21 years old, and CPR/BLS certification (preferred). Additional Duties Participate in discharge planning, conduct rounds, and review patient needs. Coordinate medical supplies and equipment, support staff training, and participate in hospital committees.
