Provides individualized and quality evaluations, assessments, prognoses, treatments and education to meet each patient's needs based on medical necessity and clinical indication, across inpatient acute care, inpatient acute rehabilitation, and outpatient services. Adheres to professional standards of conduct and ethics, maintains productivity and ongoing professional development to promote clinical excellence and evidence-based practice. Assists patients in reaching their maximum performance and level of functioning. Responsibilities

Maintains a safe, clean working environment, including unit-based safety and infection control requirements. Accurately perform individualized and quality evaluations, assessments, prognoses, treatments and education to patients and families/caregivers. Provide care in inpatient acute care, inpatient acute rehabilitation, and/or outpatient settings based on patient care and business needs. Meet weekend and holiday requirements based on FTE status. Independently assess all areas relevant to patient function, utilize sound clinical decision making skills, develop a comprehensive and individualized treatment plan, and complete all required documentation in a timely manner. Accurately enter data in Tri-City Medical Center’s Electronic Health Record System. Participate as a member of the interdisciplinary team. Provide mentorship as requested (voluntary). Follow organization’s rules, policies, procedures, applicable laws and standards while carrying out the mission, vision, and values of Tri-City Health District. Other duties as assigned by Supervisor/Manager. Qualifications

1 - 3 years’ experience as a Physical Therapist, preferred. Competent in provision of Physical Therapy modalities, interventions, and procedures, required. Knowledge and proficiency of standard office equipment and computer software (MS Windows Outlook, Excel, Word, PowerPoint, internet), required. Excellent communication and problem-solving skills, required. Excellent interpersonal and organizational skills, required. Strong customer service skills, required. Education

Graduate of an approved professional school or accredited college or university with a degree in Physical Therapy, required. Licenses

Licensed or eligible for licensure by the Physical Therapy Board of California, required. Certifications

Current BLS certification, required upon hire; American Heart Association Healthcare Provider.
