The registered nurse is accountable for the delivery and supervision of safe, quality care that is individualized for their assigned patients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning, implementation, evaluation). She/He is responsible for the assessment, treatment and care of patients in populations and ages as assigned. The registered nurse accepts and promotes professional development as an integral part of nursing practice. She/He will access, analyze, customize, coordinate and communicate the patient\'s plan of care and activities collaboratively with other members of the health care team. Families and or significant others will be included within the plan of care as appropriate. She/He will direct, delegate and manage the care delivered by colleagues and subordinates. The registered nurse performs all duties in a manner that respects and supports the family-centered care model. Collegial and collaborative care will reflect the philosophy of the Division of Nursing, the Mission and Core Values of the Sisters of Providence, and National Patient Safety Standards. Position Specific Responsibilities
Possesses knowledge of developmental needs and is competent in the assessment and treatment as it relates to the age of the patient population served. Conducts and documents a comprehensive patient assessment upon admission. Continually reassesses and applies appropriate advocacy and interventions as indicated. Interprets a patient\'s self-assessment and behavior within the context of cognitive, physical, emotional, psychosocial and maturation processes. Actively involves patient and family in the collaborative plan of care. Demonstrates and promotes safe patient care practices, including hand hygiene, patient identification, infection risk reduction, medication safety and reconciliation, falls prevention, and patient/family concerns and access to the healthcare team. Collaborates with patient, family and healthcare team to develop, document, implement and monitor an individualized plan of care based on best practice and evidence-based medicine. Provides, documents, and consults to ensure necessary patient and family education to support the treatment plan. Adapts to change and updated patient care practices within an evolving healthcare environment. Participates in discharge planning with the patient, family and team as appropriate. Performs procedures and treatments according to department/unit protocols, guidelines, standards and the Nurse Practice Act. Documents patient care activities accurately, legibly and in a timely manner, including input and retrieval of information in written and electronic systems. Delivers safe quality care in an efficient, cost-effective manner. Supports orientation of new staff and students; develops necessary training and skills for preceptor and charge nurse roles when selected. Advances professional nursing practice through continuing education, required training, certification, and participation in department/unit activities. Ensures competencies, certifications, safety information and courses are completed as required. Identifies concerns and involves colleagues and management to promote a safe patient care environment. Engages in positive communication and ongoing development of self and team; fosters open communication and patient/family-centered care and collegial relationships. Communicates adaptively based on patient/family needs; participates in at least 50% of scheduled staff meetings or signs off on minutes and contributes to meeting content. Promotes collaborative practice with other health care professionals; recognizes the impact of cultural diversity on relationships and care. Rounds with physicians and team members to promote continuity of care and patient confidence in the care team. Monitors patient status, notes changes, uses judgment and takes appropriate action; communicates SBAR status to colleagues and physicians and utilizes quality/safety tools during handoffs. Uses P.E.A.T. (Providence Early Assessment Team) when indicated by changes in patient conditions or concerns. Offers comfort and pain management measures routinely. Documentation reflects assessment, treatment and re-assessment as required to meet patient goals. Adheres to safety and infection control policies; complies with NP safety goals and emergency response procedures; meets hospital standards (e.g., JCAHO/OSHA/state and federal requirements). Participates in departmental and unit activities, quality improvement processes, tracer rounds and knowledge transfer. Safeguards all forms of confidential information; complies with organizational policies on fraud, conflict of interest and Code of Conduct. Education and/or Experience
Required - Associate\'s Degree in Nursing Required - 1 year professional nursing experience Preferred - Home Health field experience Preferred - IV and Central Line experience Preferred - Experience with wound care including negative pressure devices Skills
Mathematical Skills:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. Other Skills and Abilities:

$50.18 - $77.91 The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
