Overview The RN Care Manager is responsible for managing the continuum of care for admission through discharge for assigned patients. The role reflects knowledge of the RN scope of practice, state requirements, CMS Conditions of Participation, EMTALA, Patient Bill of Rights, AB 1203 and other regulatory requirements specific to discharge planning. The RN Care Manager partners with the multi-disciplinary team, uses evidence-based practices, data, and is aligned with the Mission and Core Values of UAMS. A hybrid remote staffing model may be available, but is not required; remote work is subject to department needs.

Summary of Job Duties The RN Care Manager is responsible for managing the continuum of care from admission through discharge for assigned patients. The role ensures adherence to applicable regulations and standards and collaborates with the interdisciplinary team to plan and coordinate discharge planning.

Qualifications Minimum Qualifications:

Baccalaureate Degree in Nursing, ADn, or Diploma RN Degree

Three (3) years RN experience with current BLS certification

Valid Arkansas Registered Nursing License

Preferred Qualifications:

National Nursing Certification

BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification

Responsibilities/Competencies

Excellent communication skills

Basic computer skills

Strong work ethics and accountability

Recruitment/retention of staff; development and implementation of evidence-based nursing practice standards

May perform other duties as assigned

Additional Information

Job Requirements: Initiates referrals, establishes working diagnoses, coordinates discharge planning with the team, documents communications, and ensures appropriate level of care and discharge timelines.

Skills: Ability to communicate with physicians regarding care needs, identify discharge delays, and coordinate with Social Work for psychosocial needs.

Participates in Performance Improvement and Evidenced-Based Practice processes.
