The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others.

Responsibilities

Patient Care: Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and applying the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards.

Assesses and evaluates patient care needs and applies critical thinking in patient care management.

Integrates relevant assessment and intervention skills in the delivery of nursing care.

Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.

Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care; delegates to others as appropriate.

Educates patients and caregivers about their plan of care, transition of care, promotion of health, and disease prevention, anticipating needs and readiness to learn.

Communicates effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.

Manages assignments within the Care Delivery Model of Modified Primary Nursing and allocates materials resources effectively.

Patient Experience

Role models Mount Sinai Relationship Centered Care through actions such as Admission Welcome, Bedside shift Report, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration.

Considers patients values, preferences, and knowledge in all aspects of care; maintains a therapeutic relationship with patient and family.

Uses evidence-based practices to enhance understanding of patient perceptions of care.

Initiates service recovery for concerns and escalates as needed to appropriate leaders or Patient Relations.

Demonstrates caring, respect, empathy, and active listening in interactions.

Supports staff use of evidence-based practices to improve patient understanding of care.

Performs Purposeful Hourly Rounding on assigned patients addressing the 4 Ps.

Quality and Safety

Practices safe, patient-centered care and escalates barriers and opportunities for improvement.

Demonstrates clinical skills and knowledge essential to quality and safety in the served patient population.

Implements process improvement strategies based on institutional evidence-based practices and procedures.

Engages in peer and institutional review processes.

Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities, and nursing research studies.

Contributes to quality, safety, and required compliance measures; applies high-reliability principles to avoid high-risk behaviors.

Escalates potential safety hazards or gaps following institutional chain of command.

Documents nursing practice in the information systems to support quality and performance improvement initiatives.

Operations

Performs charge nurse duties as assigned.

Coordinates with leadership to ensure supplies are utilized appropriately and inventory supports patient care.

Escalates material or staffing resource deficits as needed.

Demonstrates patient-focused and cost-effective approaches to care in terms of equipment, staff, supplies, and resources.

Documents accurately and thoroughly in compliance with hospital, regulatory, and legal standards.

Facilitates patient throughput on a shift basis.

Professional Development

Supports the environment of care for clinical colleagues, patients, families, and the healthcare team.

Advances clinical competence and progression from novice to expert.

Projects a professional image, communicating with care, respect, compassion, and empathy.

Incorporates ethical principles into decision making for patient and family.

Engages in lifelong learning and innovation; acquires knowledge relative to role and needs of the patient population.

Participates in shared decision making through councils and unit initiatives; serves as a preceptor for new staff and students.

Advances as a clinical leader through acting in the charge role and as a throughput facilitator.

Advocates for research and evidence-based practice within nursing practice.

Qualifications

Graduation from an accredited nursing program. BSN preferred; ADN with pathway considerations or matrícula in BSN program may be applicable per CNO approval.

Current New York State Registered Nurse license.

Basic Life Support (BLS) certification; additional certifications as applicable (NRP, ACLS, PALS) per department requirements.

Relevant clinical competence in the assigned nursing practice area.

Compensation: Salary ranges are provided in accordance with applicable law. The salary range for this role is $62.3574 hourly, with actual salaries determined by experience, education, and operational need.
