Complete Care Management
is seeking an experienced
Nurse Practitioner
to join our team in
Monmouth County, NJ . This role focuses on delivering high-quality, patient-centered care while working collaboratively with an interdisciplinary team.
Responsibilities:
Conduct thorough patient assessments, diagnose conditions, and develop comprehensive care plans. Manage and treat complex medical conditions with a focus on reducing hospital readmissions. Document patient encounters in electronic health records (EHR). Collaborate with physicians, nursing staff, and other healthcare professionals to ensure seamless patient care. Communicate regularly with patients' families and support systems. Attend facility meetings and participate in care planning discussions. Maintain professionalism and uphold Complete Care’s commitment to excellence in patient care.
Qualifications:
Master’s or Doctorate degree in Nursing from an accredited program. Active Nurse Practitioner (APN) License and Registered Nurse (RN) License in New Jersey. DEA, CDS, and BLS certifications. Minimum 2 years of Nurse Practitioner experience (post-acute care experience preferred). Strong clinical skills, including IV insertion, G-tube replacements, wound care, and suture/staple removal (preferred). Experience managing elderly patients and complex medical conditions. Proficiency in electronic health records (EHR).
