Location: Los Robles Hospital and Medical Center, Ventura, CA.
Salary Estimate:
$98,966.40 - $148,449.60 per year.
Description We have an exciting opportunity for a Clinical Rehab Specialist/Liaison to join Los Robles Regional Medical Center, part of HCA Healthcare. This role acts as a liaison between internal and external referral sources and rehab operations, establishing relationships with case managers, physicians, payors, and other referral sources. The primary responsibility is to support referral and admission growth of the rehab program through clinical assessments and screening in collaboration with the Rehab Medical Director.

Job Summary & Responsibilities
You will assist and coordinate the intake and pre-admission screening process and accurately document evidence of rehab admission criteria on Pre-Screen evaluation.
You will perform on-site and off-site clinical evaluation to assist the Rehab Medical Director in determining appropriateness of patient for admission.
You will demonstrate clinical knowledge of rehab admission criteria, leading to appropriate identification of qualified rehab patients.
You will function as primary contact for external and internal case management and referring physician and establish and maintain effective working relationships with all referral sources.
You will complete face-to-face, in-service educational contacts with physicians, case managers, other interdisciplinary members and other professionals and serve as educational resource to patients, family members and caregivers.
Qualifications
Graduate of an accredited school in clinical specialty.
Associate Degree in Nursing/Respiratory Therapy OR Bachelor’s Degree in Physical/Occupational Therapy OR Master’s Degree in Speech Therapy - REQUIRED
Current state licensure in clinical practice area or eligible for state license transfer (Registered Nurse, Respiratory Therapy, Physical Therapist, Occupational Therapist, or Speech Therapist) - REQUIRED
2+ years experience in clinical care - REQUIRED
2+ years experience in inpatient rehabilitation - PREFERRED
BCLS certification (AHA) - REQUIRED
Clinical experience in the care and management of the Inpatient Rehabilitation patient.
Experience as a Rehabilitation Specialist coordinating admissions for inpatient rehabilitation preferred.
Experience as a clinical evaluator preferred.
