Position Summary We are seeking a passionate and highly skilled Clinical Pharmacist Specialist to join our dynamic team supporting Hematology, Oncology, and Bone Marrow Transplant services. This role offers a unique opportunity to rotate between inpatient and outpatient service lines, delivering comprehensive pharmaceutical care across the continuum of cancer treatment. The position involves providing clinical guidance and expertise in the development of chemotherapy Computerized Provider Order Entry (CPOE) protocols in collaboration with information technology colleagues.
Responsibilities
Maintain interdepartmental relationships to facilitate communication related to clinical pharmacy services and patient care across the organization.
Provide patient‑ and family‑centered education and support during transition of care to discharge.
Actively participate in medication reconciliation at the patient‑care level and resolve medication‑order issues with prescribers.
Develop and implement guidelines, policies, and interventions for antimicrobial use, including the review and analysis of daily antimicrobial audits.
Review and manage medication orders for therapeutic appropriateness based on age, weight, laboratory values, diagnosis, and other clinically relevant data.
Screen for and manage clinical alerts (allergies, interactions, therapeutic duplications, ADRs) and provide interventions to meet cost‑containment and evidence‑based goals.
Communicate effectively with interdisciplinary teams, ensuring continuity of care and responding promptly to all modes of communication.
Participate in patient‑safety committees, quality‑improvement initiatives, and drug‑shortage management.
Provide education to pharmacy staff, physicians, nurses, and other healthcare professionals through in‑services and lecture series.
Serve as a clinical preceptor for pharmacy students, residents, and other healthcare trainees.
Qualifications
Education:
Doctorate (Pharm.D.) in Pharmacy or equivalent.
Experience:
Minimum 3 years pediatric pharmacy practice in an acute care environment or completion of a PGY‑1/PGY‑2 residency in pediatrics; experience with medication products, pharmacokinetics, disease states, and therapeutic drug monitoring.
Licensure & Certifications:
Current licensure as a pharmacist in Arizona (or reciprocal) and BLS, ACLS or PALS certification. Board certification in Pediatrics or Pharmacotherapy preferred.
Skills:
Proficient with Microsoft Office (Word, Excel, PowerPoint, Outlook) and pharmacy clinical information systems; excellent communication, analytical, and interdisciplinary collaboration skills; ability to precept students and residents.
Physical Requirements:
Ability to lift up to 35 lbs occasionally; routine standing, walking, sitting, and occasional pushing/pulling; exposure to bloodborne pathogens and hazardous materials in a clinical environment.
