This position provides person-centered, trauma informed services to adults with complex and/or co-occurring behavioral health needs in a community based outpatient setting. Daily activities in this position include providing individual and group therapy services, completing intake assessments; completing safety assessments; documenting care provided; attending client staffings and administrative meetings; case management; discharge planning; collaboration with internal and external colleagues. Services are provided in-person and through telehealth. This position supports and promotes the mission, vision and guiding principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to my Co-Workers."

70%- Provide direct treatment services including outpatient and group counseling and case management services to enrolled clients: 1. Develop therapeutic relationship with clients. 2. Assess psychosocial issues including social support, barriers to treatment, trauma, and substance use. 3. Provide some case management services to clients as indicated. 4. Assess client motivation and progress. Will use motivation enhancement tools to assist client in progressing through treatment. 5. Meet with clients to develop collaborative treatment plan for services, review progress in regular intervals per DHS 75 and make updates to treatment plan as indicated. 6. Provide individual, group, and/or family therapy to address needs identified in treatment plan. 7. Assist clients in crisis including assessing and developing safety plan for suicide, aggression, and overdose risk. 8. Collaborate with multidisciplinary team and coordinate client care and discharge planning as needed. 9. Refer for psychiatry and other services as indicated.

10%- Ensure program and medical record compliance 1. Complete all paperwork in determined time frame. 2. Follow DHS 75 requirements for documentation of assessment, treatment plans, reviews, transfers, progress notes, and discharges.

8%- Participates in all clinical and administrative activities as required 1. Participate in case review meetings 2. Participate in staff meetings 3. Participate in meetings with supervisor as needed

7%- Coordinate client care 1. Communicate regularly with client, other professionals, and client's support network as indicated 2.

Advocate for client as needed

5%-Will perform other duties as assigned -Knowledge of the principles and practices of behavioral health care and its application, including person-centered recovery plans, trauma-informed care, co-occurring disorders, and stages of change. -Skills in welcoming and engaging difficult to reach clients with complex needs. -Knowledge of laws, regulations, and practices pertaining to federal and state human service and behavioral health care programs. -Knowledge of assessment and treatment of mental illness and substance use disorders. -Knowledge of Motivational Interviewing (MI) principles and skills. -Knowledge of psychotropic medications, their main effects, side effects, and abuse potential. -Knowledge of psycho-social rehabilitation services. -Ability to establish and maintain effective working relationships within the agency and community partners. -Ability to communicate effectively orally and in writing. -Ability to plan and organize work effectively. -Ability to take initiative, work independently, and seek out supervision as needed. -Ability to manage one's time and emotions in an environment of rapidly changing priorities. -Excellent computer skills related to electronic record keeping, email, and internet. - Ability to maintain accurate, current records, and prepare clear and concise reports from them. - Masters degree from an accredited university in human services field. - State of Wisconsin LPC, LCSW, or LMFT required, or ability to obtain license within 3 years. - Applicants must have education and/or experience to provide SUD treatment under appropriate supervision. - Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure Policy 5.02 and 5.27. - Ability to work flexible hours, including some evenings. Physical elements: - Ability to stand, walk and sit. - Ability to exert up to 20 lbs of force occasionally or up to 10 lbs of force frequently Equipment use: - Ability to use computer and computer keyboard. - Ability to use copy/fax machine and telephone. Working conditions: - Will be divided between indoor work, community and home based visits - A valid driver's license is required - Potential exposure to physically or verbally abusive clients
