Overview

We are seeking a highly skilled and compassionate Clinical Pharmacist to join our team at Mission Community Hospital in Panorama City, California. As a Clinical Pharmacist, you will be responsible for providing pharmaceutical care to our patients in coordination with other healthcare professionals. You will also serve as a resource for drug information, educate patients about medication therapy, and monitor patient medication regimens for appropriate therapy and outcomes. Review and interpret physician orders for medications, including prerounding in the inpatient setting and ensuring medication appropriateness, dosages, and routes of administration. Collaborate with physicians and other healthcare professionals to develop, implement, and monitor patient-specific medication treatment plans. Educate patients and caregivers about medication therapy, including potential side effects and interactions. Monitor the effectiveness and safety of medication therapy, and make recommendations for changes as needed. Ensure appropriate use of medications through drug utilization reviews, therapeutic interchange, and formulary management. Verify the accuracy of medication orders and dispensing to maintain patient safety. Maintain accurate and complete patient records and medication profiles. Participate in hospital committees and provide expertise in medication-related issues. Adhere to all federal, state, and hospital regulations, policies, and procedures related to pharmaceutical care. Requirements

Bachelor\'s or Doctor of Pharmacy degree from an accredited school of pharmacy. Active pharmacist license in the state of California. Minimum of 2 years of experience in a clinical pharmacist role. Strong clinical knowledge and critical thinking skills. Excellent communication, collaboration, and patient education skills. Ability to work independently and in a fast-paced environment.
